Is It Time to Update?

MCS-150 biennial updates are due once every two years. The update due date (month and year) are determined by your DOT Number. The last digit indicates the month, while the one before the last indicates the year when updates must take place.

All U.S. DOT Numbers ending in the digit "1" must file the update in January. If you are filing a late update after the month of January and your DOT Number has not yet bee suspended, you may use this form.

Your DOT Number may not have been suspended becuase you have not been pulled over for roadside inspections at State Roads/Highways and/or were not involved in an accident resulting in DOT Audit/Compliance Review. To avoid suspension of your DOT Number and civilt penalties, file the Biennial Update as soon as possible even if you are filing late.

Deactivated DOT #

Failure to complete a Biennial Update will result in deactivation of your USDOT number and may result in civil penalties of up to $1,000 per day, not to exceed $10,000.
